Does Vincristine affect Cochlear function in children with acute lymphoblastic leukaemia?

Context: Vincristine chemotherapy has dose dependent ototoxicity. Early detection of ototoxicity is better with otoacoustic emissions and high frequency audiometry than conventional pure tone audiometry. The study was done to see if vincristine treatment interferes with hearing sensitivity in children. Methods and Material: A prospective study was conducted on twenty-three children with acute lymphoblastic leukemia (ALL) on Multi Center Protocol (MCP 841).These were subjected to conventional audiometry, high frequency audiometry and distortion product otoacoustic emissions (DPOAEs) before starting chemotherapy. The follow up audiological evaluation after early intensive phase chemotherapy (approximately 6 months) was conducted in thirteen children, who received 12 doses of vincristine (1.4 mg/m2), cranial irradiation of 1800cGy (>3 years) as per protocol and antibiotics as per clinical demands. Results: Baseline audiological evaluation was normal. Follow-up evaluation DPOAEs showed a declining tendency, however changes did not reach statistical significance. Differences in median hearing thresholds prior and post treatment in higher frequency audiometry were also minimal which was not statistically significant. Conventional audiometric thresholds were not altered. Conclusions: The reduction in the signal noise ratio of DPOAE, and reduced hearing sensitivity in high frequencies in post chemotherapy in comparison with baseline measures cannot be ignored though it has failed to reach the level of statistical significance. children on vincristine should have a pre chemotherapy and follow up audiological evaluation with DPOAE The results of the present study needs to be strengthened by including larger sample and long term follow up.
Perception of Spectral Ripples and Speech Perception in Noise by Older Adults

Present study aimed to obtain spectral ripple discrimination threshold in older listeners and to correlate it with their speech perception abilities. In experiment I, fifteen older adults and fifteen young adults with normal hearing sensitivity were tested for spectral ripple discrimination ability in quiet (SRDT) and speech recognition in noise (SNR50). In experiment II, twelve older adults with normal hearing sensitivity were tested for spectral ripple discrimination ability in noise (SNR-SRDT) and SNR50. SRDT and SNR50 of older adults were significantly poorer than young adults. There was a significant negative correlation between SRDT and SNR50 and significant positive correlation between SNR-SRDT and SNR50. Linear regression analysis revealed that SRDT accounted for 21 % of variance in SNR50. Quadratic regression analysis revealed that SNR-SRDT accounted for 63 % of the variance in SNR50. Despite having normal hearing sensitivity, older adults exhibited reduced spectral resolution and poor speech recognition in noise. Poor speech recognition in noise in older adults could be partly due to their spectral resolution abilities
Respiratory swallowing coordination in healthy term neonates during the first 4 days of life

This is a cross-sectional, observational study evaluating the coordination between respiration and swallowing in 22 healthy term neonates from birth to 4 days of age. Methods: Respiratory swallowing coordination was measured using Kay digital swallowing workstation by placing the nasal cannula at the entrance of the nares during breast feeding. Swallowing apnea duration (SAD) as well as the percentage of distribution of swallows in each respiratory phase such as mid-expiration (EE), shift from inspiration to expiration (IE), shift from expiration to inspiration (EI), and mid-inspiration (II) was evaluated. Results: Results showed that SAD in neonates is lesser than that of adults. Neonates preferentially swallowed at mid-expiratory phase which was followed by the transition among IE, EI, and the mid-inspiratory phases. The predominant respiratory swallow phase seen in neonates is same as that of adults. Conclusion: The findings of the objectives of the study gave insight about the temporal relation between respiration and swallowing in neonates and forms the basis for comparison against disordered respiratory swallowing coordination in neonates at a risk for dysphagia.

Ageing effect on air-conducted ocular vestibular evoked myogenic potential

One of the recent diagnostic tests to assess the function of otolithic organs is through vestibular evoked myogenic potential (VEMP) testing. There are equivocal findings on effect of aging on ocular VEMP (oVEMP) parameters with reference to latencies. Hence this study was taken up to investigate the age related changes in oVEMP parameters. This present study considered 30 participants in each age group i.e., young adults, middle-aged adults and older adults. oVEMP were recorded using insert earphone at 100dBnHL at 500hZ short duration tone burst. The results showed in older adult significant difference in response rate, latencies and amplitude as compared to young and middle adult. Hence age should be taken into consideration when interpreting oVEMP results.
Speech Motor Control in Children with Downs Syndrome: Evidences from Formant Centralization Ratio

Introduction: Studies have highlighted the sensitivity of Formant centralization ratio (FCR) in the identification of vowel centralization. Since children with Downs Syndrome (DS) are found to exhibit imprecise articulation, the present study aimed at evaluating whether children with DS exhibit vowel centralization using FCR. Method: The study included 30 children with Downs syndrome and 30 healthy age and gender matched controls in the age range of 6-8 years. Materials consisted of 9 words with three vowels (/i/, /u/, and /a/) in the initial, medial, and the final word positions respectively. First and second formant frequency values were extracted using PRAAT software. The vowel-formant data (e.g., frequency of F1, F2 etc.) were separately averaged for each child and then the FCR was constructed from these averages using the formula (F2u+F2a+F1i+F1u) / (F2i+F1a). Independent t-test was employed to find out the significance of difference between the means of the two groups. Results: The results of Independent t-test revealed significant difference between the mean FCR of both the groups at t= 8.535, p < 0.00 indicating higher FCR values in children with Downs syndrome in comparison to typically developing peers. Conclusion: The results of the present study indicated that FCR obtained from both the groups vary significantly, thereby indicating that the vowel production in children with Downs syndrome may be subjected to vowel centralization. The present findings suggest the potential abilities of FCR metric in identifying vowel errors without characterizing the nature of vowel abnormalities and hence this metric should be used in the clinical population with caution.
Vowel harmonic amplitude differences in individuals with unilateral vocal fold paralysis

Background: The studies reviewed suggest the potential clinical applications of vowel harmonic amplitude differences. In this study, it was hypothesized that there would be abnormal reduction of higher harmonic amplitudes relative to the amplitude of the first harmonics in individuals with unilateral vocal fold paralysis (UVFP) as they are characterized by breathy voice due to inadequate closure of vocal folds.Method: A total of 80 participants were divided into clinical group and control group. They were instructed to phonate/a/at their most comfortable pitch and loudness. Fourier transformation of the recorded acoustic signal was first performed to create a spectrum. Amplitudes were measured for the first and second harmonics as well as the harmonics at the first, second, and third formants using the Computerized Speech Science Lab. Results: Independent t-test was employed to compare the significant differences between the clinical and the control group in both males and females. The results revealed significant differences across the two groups and gender at P < 0.05. The obtained results were discussed with respect to the underlying pathophysiology. Conclusion: Spectral deviations in the clinical group are explained due to the presence of phonatory gap due to the UVFP.
